In accordance with financial regulations, ensure that the letter from your financial institution includes each of the following requirements.
- A statement from your financial institution that the credit card / bank account is active and can accept payments.
- Must be on the financial institution's official letterhead.
- Include bank account / credit card holder's name.
- States the account type.
- Last four digits of the credit card or full ACH information (routing and account number).
- Email address of the bank
