Cox Enhanced Lifeline offers basic phone service for only a dollar per month to qualified low income customers who participate in at least one qualifying state or federal programs and who live on current or former reservation or tribal land, as defined in the Code of Federal regulations (Title 25, Sec 20.1, Para v).
Reservation or tribal land is defined as all of Oklahoma State except for the panhandle and the southwest area identified as "the Leased District." Therefore, the following counties should be excluded from participation in Enhanced Lifeline program: (1) the panhandle, Cimarron, Texas, and Beaver counties; (2) Southwest corner of Oklahoma, Greer, Harmon, and Jackson counties, as well as the area of Beckham County south of the North Fork of the Red River.

Restrictions
- Enhanced Lifeline applies only to a single telephone for the principle residence of eligible households.
- Applicant must meet the requirements of a state established income test.
- Applicant must not be dependent for federal income tax purposes, unless applicant is more than 60 years of age.
- Name on phone bill must match name of the participant in eligible program.
- Eligibility is reviewed periodically. If the customer no longer meets eligibility requirements, lifeline benefits shall be terminated.
- Enhanced Lifeline customer must notify Cox in the event that he/she becomes ineligible for such qualifying benefits.
- Each year, the customer must renew his lifeline connection. Cox mails out renewal forms to customers, and if the customer fails to mail back the form, the lifeline benefit is terminated.
- If the Enhanced Lifeline customer becomes non-pay i.e. does not pay his bill, service may be terminated.
- Activation of benefits begins when proof of eligibility is received. If the customer wishes to activate his phone service prior to approval & processing of required forms & documentation, the customer will get charged regular tariff until such time that his forms/documents are approved and processed.

The above application required the Enhanced Lifeline applicant to self certify under penalty of perjury that 1) he/she is a resident on Tribal Lands or former tribal land, and 2) that he/she is eligible for, or is receiving benefits from, one or more of the qualifying programs and must indicate which program(s).
